If you are a new patient, please complete the online registration form prior to your visit. If you would prefer, hard copy paperwork can be completed in the office. It is required that you arrive 15 minutes before your scheduled appointment to fill out paperwork. We have a 15 minute late grace period (15 minutes past scheduled time if paperwork has already been completed) otherwise we will kindly ask you to reschedule your appointment.
